Home and Community Services

Home and Community Services include helping people gain, maintain and improve skills that allow them to live and participate in their local community.  Service Coordination

Service Coordination is the active assistance offered persons as they navigate the community and various service systems in pursuit of the necessary and desired services and supports that will assist them in achieving or maintaining their personal goals.

Transportation Services

Direct Service and services to provide transportation for individuals enabling them access to services/activities in accordance with their Individual Service Plans.  These services enable individuals to gain access to their community.

Pre-Vocational Services

Pre-vocational training and services prepares individuals for paid employment.  Both our Vocational Rehabilitation Program and School-to-Work programs meet many of the needs in this area. 

Respite Services

Respite Services are direct services that are provided to supervise and support individuals on a short term basis because of the absence or need for relief of those persons normally providing care.
